a. Why you are interested in providing Residential Services to SARC- sponsored individuals;
b. Your background and experience working with ID/DD individuals;
c. The county or counties within the SARC catchment area in which you intend to provide services;
d. the steps you have taken in preparation to provide services to SARC sponsored individuals; and
e. how you will implement and support SARC-sponsored individuals’ preferences and choices as contemplated by the HCBS Final Rule and Title 17 of the California Code of Regulations.
[Please Note: We welcome existing vendors who wish to obtain refresher training. If you are attending as a refresher, please include this information in your Letter of Intent.]
2. Your updated resume.
3. Your DSP Year 1 and Year 2 certifications.
4. Your Administrator Certificate.
